Click the Enroll Now button to start your registration process. We will ask you to enter your information, confirm your eligibility, and select your condition.  You will be directed to download the Onduo app to get started.

Once you register, you’ll be asked to enter your information, confirm your eligibility for the program with your insurance, and select your condition. You will then be directed to our partner, Good Measures, to create your account and download the app.

In most cases, Onduo is provided by your employer or healthcare plan at no additional cost to you.⁵ If you are on a high deductible health plan, there may be out-of-pocket costs associated with some components of this program.  If you have any questions about your benefits and costs for you, check with your benefits administrator or call the phone number on the back of your insurance card.

The Healthy Weight program is available at no additional cost to eligible members.⁵

To be eligible you must be:

• At least 18 years old
• Diagnosed with type 2 diabetes
• Covered by an eligible healthcare plan

To be eligible you must be:

• At least 18 years old
• Diagnosed with high blood pressure/hypertension
• Covered by an eligible healthcare plan

You are eligible for the type 1 diabetes program, unless you:

• Are under 18 years old
• Are pregnant
• Have not been diagnosed with type 1 diabetes
• Or are living with the following conditions
  • Kidney failure
  • Liver failure
  • Have had a transplant

     

To be eligible you must be:

• At least 18 year old
• Covered by an eligible healthcare plan
• Anyone interested in losing weight or making healthier lifestyle choices

Onduo collects member data directly from members’ connected devices, and from any integrations the member opts to use. The data is used to understand your health and health risk factors, track progress, measure outcomes, and deliver meaningful insights.
